"100% Access and Zero Disparities"

For the uninsured, gaining access to quality healthcare can be challenging. Even though public hospitals, health departments and community health centers exist to help the uninsured, providers of these networks experience difficulty coordinating services amongst one another which result in duplication of efforts and less direct patient care. The ACAC links clients and these collective services to tear down the barriers that have historically hindered people receiving care.

The Atlanta Community Access Coalition was created to develop a community-based health system that improves access to health care services throughout Fulton County for uninsured and underinsured residents. Funded by a grant from the Health Resources and Services Administration (HRSA), the ACAC achieves this goal through the use of an advanced case management system that allows partners to share health and psychosocial information for each client.
